The PATHWAY project aims to transfer and implement a mature, personalised neurorehabilitation pathway from an experienced European centre to a receiving institution by focusing on organisational and clinical integration rather than developing new technologies. Its main goal is to enable the routine delivery of personalised, data-driven rehabilitation through structured patient stratification, scalable supervision models (e.g. 1:1, 1:2, 1:3), and the integration of validated digital tools into coordinated clinical workflows. The project emphasises building institutional capacity through training, knowledge exchange, and the adaptation of standardised procedures to ensure sustainable implementation within existing healthcare systems. Ultimately, it seeks to improve access to high-quality personalised rehabilitation, increase efficiency and resource use, and create a scalable model that reduces disparities and supports wider adoption of personalised medicine across Europe.
